For many people considering full-mouth implants, the biggest struggle isn’t just missing teeth, it’s the uncertainty around the process itself. Patients often feel overwhelmed by questions about bone loss, treatment timelines, travel requirements, and whether they’re even a candidate for a long-term solution. In this expert Q&A, Dr. Sevilla breaks down the most common misconceptions, explains how STAR Dental determines the right implant approach, and shares what patients are truly hoping to regain beyond the clinical outcome: confidence, comfort, and life without limitations.
How do you determine if a patient is suitable for All-on-4, 5, or 6 implants?
C.S.: At STAR Dental, this decision is driven by bone quality, bone volume, and long-term stability, not a one-size-fits-all approach. We also consider the patient’s age and how long the restoration needs to last, as well as the current condition of their teeth and oral health. Many patients come to us with worn or failing teeth from years of grinding (bruxism), periodontal (gum) “disease”, and related bone loss, multiple failing restorations, or missing teeth. These factors play a critical role in determining the most predictable, long-term solution.
So, we start with a CBCT scan to evaluate 1) available bone in the upper and lower jaw, 2) bone density and “strength”, and 3) bite forces and functional demands. From there, All-on-4 is typically used when bone is more limited, using angled implants to maximize existing “bone”, and All-on-5 or All-on-6 are recommended when more bone is available or when additional support is needed for long-term durability. Especially in younger patients or those with heavy bite forces. Our philosophy? Use the fewest implants necessary but never compromise strength, function, or longevity.
What’s the biggest misconception patients have before starting full-mouth implant treatment?
C.S.: The biggest misconception is that once the procedure is done, they’ll no longer need dental care. In reality, dental implants require ongoing maintenance to ensure long-term success. Regular cleanings, exams, and x-rays are essential to monitor implant stability and overall oral health. Annual cleanings and a panoramic x-ray are especially important—not only to confirm everything is stable, but also to maintain the lifetime warranty on your implants and evaluate how well you’re caring for them at home.
Another common misunderstanding is that the entire process is completed in just one day. While patients do leave with fixed teeth the same day, the full treatment process takes time. The implants must properly integrate with the bone, and the gums need to fully heal before the final teeth are placed. Without adequate time for the implants to integrate, there is a significantly higher risk of implant failure. Without proper gum healing, patients can experience food traps, which can lead to inflammation and possible bone loss around the implants.
Most full-mouth implant cases can be completed in just two primary visits to Costa Rica.
For those who’ve been advised to get bone grafting, how do zygoma implants simplify the process?
C.S.: Zygoma implants are a game-changer, especially for patients with severe bone loss in the upper jaw. Instead of multiple bone graft surgeries, 6–12+ months of healing after each graft, and dealing with unpredictable graft success, zygoma implants anchor into the zygoma, or cheekbone, which has dense, stable bone. This eliminates the need for grafting in many cases and allows for immediate function with faster treatment timelines. This turns what could be a long, multi-stage process into a streamlined solution—often completed in one surgical phase.
Typically, how many visits does this treatment require from start to finish?
C.S.: STAR Dental is structured around efficiency without compromising quality, especially for traveling patients. Most full-mouth implant cases are completed in two primary visits. The first visit typically lasts 5 days and 6 nights, and includes the clinical evaluation and smile design, cardiologist and bloodwork clearance for IV sedation, surgery for implant placement, delivery of fixed temporary teeth, as well as the post-op and bite check. This is followed by a 4–6 month healing phase, during which osseointegration takes place as the implants integrate with the bone. The second visit usually lasts 9 days and 10 nights, and includes the clinical evaluation to confirm the implants’ integration into the jawbone, a try-in model of the final teeth to confirm fit and aesthetics, placement of the final custom-designed prosthesis, and the bite check with night guard delivery. This minimizes travel while ensuring optimal healing and precision in the final result.
What patients are really seeking is a chance to feel like themselves again.

Beyond the clinical side, what’s the biggest life challenge patients are hoping to overcome with this treatment?
C.S.: For most patients, it goes far deeper than teeth. They’re trying to reclaim confidence—smiling, speaking, and being seen without embarrassment—freedom to eat what they want without pain or fear, and a sense of normalcy where they no longer have to plan life around dental limitations. Many patients come in feeling socially withdrawn, frustrated with failing dental work, and simply tired of temporary fixes. What they’re really seeking is a permanent reset—a chance to feel like themselves again. And what they receive after All-on-4, 5, or 6 treatment is truly life-changing: a fixed, stable set of teeth that looks and feels natural, the ability to eat comfortably and enjoy food again, renewed confidence in social and professional settings, relief from chronic dental pain and constant dental repairs—often accompanied by improved overall health as a result of better oral health—and a long-term solution that restores both function and quality of life. It’s not just a new smile. It’s a return to living without limits!
As Dr. Sevilla makes clear, full-mouth implants go far beyond the clinical result. They help restore confidence, comfort, and a renewed sense of normalcy. With a treatment journey built around precision, healing, and efficiency, patients can move forward with confidence and step back into everyday life with ease.
